Need opinions from high IQ looksmaxers on what to do about my chin, please.

If you remember my silicone chin implant was either misplaced or migrated upward:

I've already spoken with a well known plastic surgeon here about options and he said he wouldn't touch my chin until this was removed and I wait 6 months.

His reasoning was that he wanted to see what my pre-op base chin looks like before performing another implant or genioplasty.

He also said fixating or using screws on silicone implants is not possible. (What the fuck?)

I'm booked in to see an actual Maxfac next Monday and I'm going to ask if he can do removal + genioplasty during the same procedure as less surgery is better IMO. (I'm also sick to death of surgeons not being able to execute implants correctly.)

Do you think the original surgeon had any merit suggesting removal + 6 months of waiting?

I think it's bullshit. He should know exactly what he's working with based on imaging and what he see's once the implant is taken out.

It's bullshit he can see everything through x-rays. Why don't Just get a sliding genioplasty?
 
It still astonishes me that a surgeon can spend years of his life studying his craft, have decades of experience doing the same procedures over and over, and still fail at the simple task of putting a piece of plastic on your chin.

I think he's full of shit and makes me doubt all his "experience".

Some surgeons are literally butchers with a legal license.

That's the plan, but he didn't even want to do that until I've had removal + waited 6 months.

Seems like unnecessary time and trauma to my face.

Completely unnecessary in this case, even harmful as it gives the tissues in chin an opportunity to atrophy. Find someone who is confident at removal and replacement/genio at the same time.

Of course, you can perform a genioplasty at the same time as you remove the chin implant, it is even recommended imo. Beware that a genioplasty may give you indentations where your mandible was cut and moved forward. The more you move the chin forward and downward, the greater chance you will have visible indentations.

I can understand his logic , but I am sure a Maxfac can give an indication using your scans and common sense . Always go to a maxfac for jaw related work , unspecialised plastic surgeons are fucking clowns.

You don't want to undergo more procedures than necessary, try to find someone else. I had chin implants removed twice by new surgeons while simultaneously having other work done on my chin with no major issues. One time with medpor which is a lot more difficult to remove. So it can definitely be done.

I'm legit about to leave to speak to a more ethical maxfac surgeon in the city.

As someone on jawsurgeryforums pointed out, the surgeon I spoke too was likely far too concerned about his realself reviews and image to undergo a revision.

Genioplasty:

- Does nothing to widen your chin (Unless modified. No surgeon is willing to do this for me)
- Limited to 7-8mm (So I'm told in my case)
- Results in step-off (Noticeable void created after chin is slid forward)
- Must be done via intra-oral incision (More trauma to this area can result in lower lip droop, chin ptosis)
- Creates overly prominent mentolabial fold (Case by case basis)

Implant:

- Also permanent (If correct material / fixated)
- Completely bespoke / customizable
- Easily widen chin (Lateral wings add width all the way along mandible)
- No size limit (Within reason)
- Can be done via submental incision (Little to no risk of lower lip droop / chin ptosis)
- Adds MASS (Doesn't simply displace)
