Now, about dosing. The way PTH analogs work is pretty unique. Unlike most bone drugs that slow breakdown, teriparatide and abaloparatide stimulate new bone growth by mimicking the body’s natural parathyroid hormone but the key is how they’re administered. These drugs need to be injected once daily, creating a short burst of hormone that tells your bones to build more tissue. Continuous exposure, like constant high blood levels, actually causes bone breakdown instead of growth. So timing and dosing are everything.
For therapeutic use, the standard dose of teriparatide is 20 micrograms per day injected under the skin, while abaloparatide is typically dosed slightly higher at 80 micrograms daily. Treatment length is capped at 18 to 24 months to avoid potential risks noted in animal studies, although in humans this risk remains theoretical and extremely rare. This means most patients get a year or two of treatment at most.

One important note: PTH analogs require careful monitoring. Because they stimulate calcium release and uptake, you need to keep an eye on calcium levels and kidney function. Supplements like vitamin D and calcium can help balance this out.
