Your maxilla has to stretch forwards as far as it can to try and catch up to the lower jaw, and even then it fails to keep up, falls behind, and you get an underbite.

Just because you were born with an abnormally short anterior cranial base.

Short anterior cranial base is, in the following image, when the distances from the "Ptv" point to the "Or" point, and from the "Ptv" point to the "Na" point, are short.

Short cranial base is an unfixable falio.

The underbite that results from it can be compensated for by bimaxing the patient, which may dogmaxx them if done to exess, ruin their nose shape, etc. In severe cases, the patient’s maxilla can’t be advanced enough to correct their occlusion, so they need premolar extractions from their mandible to recess their jaw. Brutal situation.

The 2nd guy’s ear-to-forehead distance is short. His maxilla is recessed as well. Short cranial base, slightly recessed maxilla, and maybe slight mandibular overgrowth all are contributing factors to his underbite.

What a strange abnormality
It's not strange. It's maxillary hypoplasia.
Has an occurrence rate of about 5-10% in caucasians, probably higher if you take into account downward grown class IIs.

Yes, but a person can also have a normally grown maxilla attached to a recessed frontal cranium.

That's 100% true but often they are part and parcel.

Robert Pattison. He has a decent frontal cranium but a hypoplastic maxilla.

When he was younger, looking at his front you couldn’t really tell but he’s aged quite poorly. Maybe that was the cause of it ?

It's definitely a factor. A good maxilla provides critical support to soft tissue.
However, you have the likes of Kieran Culkin - who has a somewhat similar problem - and he still looks 25 at 40 years old.

So the short answer is, yes a robust maxilla is key to age proofing but soft tissue genetics is very important as well.
