I’m brainstorming a potential surgery plan, would like some input

Here’s my skull:

Brootal, ik

The plan is as follows:
-
Advancements are to be made with 6-7 mm in mind

Normal BSSO
Nothing special here, just bsso since I am recessed in both jaws, the chin just covers up jaw recession.

Quadrangular Lefort I
The real game changer. Modified to include a portion of the upper maxilla, alongside the cheekbones. Included will be a downgraft (sfscels unite!) and of course cw rotation.

T Genioplasty
The only part I’d like to remove bone in. My chin is prognathic, as can be seen in the x rays. A T genioplasty is in way to reduce width, helping balance out the chin with the rest of the jaw, and a potential backsliding genioplasty to reduce length.

Miscellaneous
VY closure could also be used, just to help with lip projection. Also looking into an alar cinch to prevent the nose from widening. In general other things that can be added I’d like to hear.

I had a whole pdf documenting the specific changes but I’m too lazy to get it right now. This is the plan for now. Feedback appreciated :owo:

lefort 1 with implants is superior to quadrangular lefort. And you might not need BSSO because you are not recessed

How so? My understanding with implants is, sure, in pictures and emotionless it looks great, definitely mogs quadrangular lefort. However, especially with paranasal implants, when you start talking and smiling, it begins to look uncanny. Not to mention I eventually plan to return to martial arts about a year after surgery

a bit of SFS?

Then i suggest you try extruding with orthodontics instead of jaw surgery
Very legit option with much more predictable outcome

Recessed maxilla to recessed maxilla

It’s not just about the smile

You're probably not nearly as recessed as you think
I used to think I wasn’t either, until I saw a higher definition x ray:

Jaw maybe not as much, but the maxilla is for sure recessed. Also it’s not only about the forward growth, I’m vertically deficient in my jaw

Personally i wouldn't do any martial arts after the maxilla has been downgrafted and advanced. Sounds retarded.
