there are hundreds on lefort results without philtrum slant
In the pic you showed, the philtrum is slanted at the same angle in the before/after

I am thinking the guy in the OP needed clockwise rotation

feel free to prove me wrong about this, but I've been assuming that the height of the cut depends on how long your maxilla is, and if you had a longer maxilla, you'd likely get a cut that doesn't reach your nose. Would be interested to see his CT scans.

That's what I'm thinking, if your maxilla is slanted like that naturally but not so noticable due to being recessed, then you advance it forward it becomes more noticable as it presses more against the soft tissue
