A failo is a feature/ratios negatively impacting your face.

So it can be conscious and subconscious.
It doesn't necessary have to be noticeable like recessed. When its already convincingly noticeable that's already a major failo
They best way to know is ask others to rate no matter how great a rater you personally are.

Then photoshop it to ideal range. See if you ascend then ask other people to check it
Ratios varies across anatomy and some people do look better with a slightly longer mid face like 0.95 but it won't deviate too much.

Change the feature with an app and confirm thats the best way. For example I did the same for my esr and all raters agree my proportions went better then my current esr is a flaw and needs an OBO

Dude I get that ur really self conscious about your esr, but it's really not good for your mental health if you keep making threads about it. Many have already told you that if it isn't noticable, then it isn't a failo

Then you need bone shaving your zygos face too wide according to ur description.

If ur mid face is like exactly 1:1 then its ok for ur absolute spacing

Not ok tbh. 0.45 is already slightly bad on some cases but for ur case you don't need an OBO just a shaving will fix it
