Which surgery route should I pursue

I’m thinking of starting off with either a fat graft or something else to ascend my eye area, and then if I’m satisfied with whatever results that gives I’ll stop hardmaxing there. Unless you guys think I should try and fix my midface/lower third instead, but eyes, imo, would be the easiest for now. Anyways, what would yall recommend?

My biggest flaw (from what I understand, although my knowledge isn’t that great) is my underdeveloped midface, especially in proportion to my brow ridge and chin, giving me a concave look. Fixing this would be ideal, but I’m not sure if I’d be willing to commit to something as extreme as a bimax/trimax to fix it, hence why I’d rather just fix my eye region and see how I feel then.

Last point, about 15-17% bf right now, working on leaning out. Also training masseters, maybe that’s cope but I’ve seen improvements over the month-ish I’ve been doing it.

Forgot to mention, I’m squinting in the first pic, and you can also see my underdeveloped maxilla and overprojected chin more-so when I smile, as seen in the third and fourth pics.
 
Work on eye area: infraorbital implants and periorbital fat grafting. Fat grafting alone won't be enough, cause it ain't truly a thin fat pad tissue. Any other procedures not worth the pursuit/ROI

But first thing get lean, to accentuate your lower third. You over all jaw development/occlusion doesn't seem off, like you suggest.

Depends on your expectations. Most people think/want to fix the undereye just through getting infra implants, hence the designer/surgeon makes/uses too big/bulky designs, which can result in an uncanny look.

Infra implants should ideally be very minimally noticeable, and for the final result you'd need additional refinement with fat grafting.

So ultimately it just comes, whether you want maximum ROI, or ideal/logical course of action.

I have a bit of a similar under-eye to you, and plan to get infras and then periorbital fat grafting.

1) will you post an update of your own process?
2) are you doing it locally or out of country?
3) so since implants should be mixed with fat graft, is that meant to be staged, or back to back?
4) in my situation, what should I tell my surgeon I want?
 
If you do the fat graff with canthoplasty for your canthal tilt you would mog bro

Alright, I’ll look into cantho more, I’m not as researched on it. Any threads of post op? Looking online, most results look uncanny.

even canthopexy may be enough in which you may be able to do fat grafting and it in the same session under local avoiding the anaestia costs. But regarding your jaws it dosent really matter, i feel like, because genio and lf1 would make sense but then it would fuck up your bite so i would just not touch it. But to me your biggest issues are your lips maybe lip lift and lip filler is likely a good idea because your lips are very small

1) Yes, I plan to do that, since my case is an obvious deformity/function case, and is relatively complex/rare. Would do for a good documentation.
2) I got MARPE + braces locally, but the other procedures OBO/rhino + possibly infras are gonna be done in Turkey, hopefully next Summer.
3) Staged.
4) Depends whether it's a plastic of MFS surgeon. But just tell the truth, regarding your concerns with eye area, and that you want to increase undereye support, by adding conservative infraorbital implants, and plan to do periorbital fat grafting in the future as well.

Regarding the canthal tilt, I personally wouldn't touch it, cause it's pretty much neutral, and the importance of PCT is overrated. YES, it will increase your objective PSL, but too much of a PCT = low trust / unappealing for females. So, up to you to decide.

Going to be completely honest, I’ve never even heard of a canthopexy until now. Looking into it, it definitely is an extremely viable option for me. I would want to do 2 procedures for my eyes though, since just a pexy won’t fix my undereye darkness, there would need to be an implant or fat graft. Adding tilt would be ideal but my main priority is adding undereye support, I think I’d rather go the route of the other commenter and go infras + fat graft.

Regarding jaw, you’re right. The way my face is built, a lf1 would be really transformative, but it would mess up my bite so I’d need bimax, but then that would overproject chin so I’d need trimax 🤦‍♂️ hence why I’d rather start small and fix eye region, then move on to that if I want to keep climbing, but at that point it’s so much extra risk for low ROI. It does suck since smiling nukes my proportions but I’d be happy getting to MTN 🤷‍♂️

I want to do something for lips but I’m extremely sus of filler, I’ve seen too many migrations to consider it. Maybe fat graft.

1) what’s the deformity? I’m wondering since you said our cases our similar, but mine isn’t deformed. Anyways, let me know if you ever post about it, I’d love to see the documentation/update (I’m not sure if you can add on here, I just recently signed up).
2) Thoughts on MARPE? I’ve been considering it, my smile is fairly narrow, would be nice to widen palate. Good luck on your process though.

You’re right about tilt btw, I’m not too concerned about it. To me, high risk of looking uncanny for low reward of a small increase in looks. I don’t care too much for it

I never really noticed my lips, if it’s that bad then no problem, I can do that before any cantho or infras. I’m worried about filler though, heard it’s extremely high risk of looking uncanny/migrating. Fat graft better?

I wouldn't say they're always cope but don't get infra, unless your super low bone mass which u aren't. If your lucky it can look very good but it's very invasive, and so many things can go wrong to the point where the structure of your undereye area is permanently damaged with pain and deformation. Just do high end fat grafting if you care at all, wont be as affective but is way more consistent & helps a little, infras as of now are more experimental and are a gamble.

Hmm, interesting. This honestly is one of the biggest factors delaying my plan for surgery, the discrepancies. On one hand, people are telling me to for sure get light infras + fat graft (and I see the same on other posts), then other times I see the opposite, saying just get fat graft.

What do you mean by ‘not as effective but more consistent’? As in, not as profound of a result, but the result is more sure to be what’s shown to happen?

As of right now, this is my biggest internal debate for fixing eyes. Infras + fat graft, just fat graft, cantho + something else. So many choices lol

1) My deformity has to do with hemifacial microsomia (one side less/more developed than the other). The under eye area has nothing to do with it. It's just that my undereye is similar recession wise to yours.
2) Need to consult and ortho, get CBCT, cephalometry scans and measure the inter molar width. 38+mm is considered normal, 42+ is optimal, mine was 35.5mm. After expansion it's currently over 41.

Yeah, wouldn't pursue cantho myself tbh.
