Am I a BIMAX candidate?

Hello,
(Pictures at the end!)

I’m 19 and have been looking into orthognathic surgery for a while now. I’m trying to figure out whether bimax (possibly with some CCW rotation) would actually make sense in my case or if I’m overestimating what surgery could do for me.


My main aesthetic concerns are:


  • I want a wider and stronger-looking lower third.
  • I want more forward growth.
  • Getting rid of or reducing my nasolabial folds is a big priority for me.
  • I feel like my maxilla might be somewhat recessed, but I’m not sure if that’s actually the case or if I’m just misdiagnosing myself.

I’ve attached front and side profile photos.


I’m interested in hearing honest opinions from people who have had surgery, done a lot of research, or understand cephs and facial structure. Do you think I look like someone who could benefit from bimax and/or CCW rotation, or do you think the improvement would likely be minimal?


Also:


  • Do you see any obvious maxillary deficiency?
  • Does my lower third actually look narrow to you?
  • Would advancing the jaws realistically help with my nasolabial folds?
  • If you were consulting a surgeon, what movements would you expect them to consider?

I’m looking for honest feedback, whether it’s “yes, you’d benefit” or “no, I don’t think surgery is justified.”

probably just get a lateral ceph and measure your SNA SNB etc, it will give you more insight on whether any surgeon would be willing to perform the surgery on you, since that's what matters at the end of the day if you're unhappy with your looks. i am sorry if this is unhelpful

Not unhelpful at all! Will probably get those measured if I actually plan on getting a consultation. But even by just looking at my face from outside you can clearly see some recession in the maxilla. From the side it looks sunken in wouldn’t you say?

How do I get my maxilla forward then? I know its not a class2/3 case but my palate is narrow and causes me to mouthbreath. And it’s just ugly.

if its because of narrowness then u can address it by getting marpe but u should get it measured by an orthondotist first

But bimax will simply advance my upper maxilla forward and upward as well as bringing my chin/mandible forward. So how does that not benefit me when my lower third is my biggest failo?
 
actually looks like palate expansion might be better in ur case. with some augmentation to the lower jaw

can you tell me more about it? What would you do if you were me? Get marpe and then bimax?
 
if your goal is a wider lower third and less nasolabial folds, then you can forget about bimax.

this is strong af.

-ur palate is definitely a bit arched though, so expanding is would be a great step. it widens the roof of ur mouth and lowers it, flattens it out. the space behind ur nasolabial folds gets a bit more volume so it can reduce those. if it doesnt, you could get off the shelf paranasals augmented there, but i never see anyone do those so idk. fillers, fat grafting, and face lifts are other options

-chin goes down a bit. id personally do a genio and move it a few mm forward and upturn it a bit. do this after braces and palate expansion though, in case the changes to ur bite shift the chin laterally so u can recenter it as well

-then any form of jaw augmentation. a bit of width there would benefit u. could be easy off the shelf implants, custom implants, fillers actually work decent there ive heard, or some full wraparound design that may even help u skip genio; people seem to complain about these a lot though

But very useful information. Thank you! I’ll look expanding my palate. I was actually at a consultation earlier this year and the woman told me she didn’t think I “needed” expansion and that my palate was good enough.
What should I say at the next consultation with her or someone else for the highest chances for getting accepted.

If you want to get it covered by insurance as an adult ur probably screwed. you'll probably have to find a way to do it out of pocket. i was told the exact same thing even though my intermolar distance is 38mm with severe tipping, and i have obvious speech issues. "modern" orthodontists and insurance consider high 30s a good with even though ideally anything below mid 40s needs correction most of the time

ur zygos and mid to upper maxilla are very well developed makes me wonder why the lower part would be weak. had braces before teen years?

I will find a way 😂but yeah the orthodontists in my country are all dumb and think having straight teeth is all that matters

I had braces at early teens. Had very crowded teeth and super narrow maxilla. And instead of expanding the maxilla and fixing the issue the dentist recommended to get braces of course🧠

same shit happened to me 🤦🏼 sucks most orthos just do whatevers easiest to straighten the teeth instead of fixing ur structural issues. and most parents dont know any better

Yeah that sucks…
But if I get my palate expanded which will take a few months. Then I’ll need braces again to align the bite. But what would be the next step to fix my narrow lower third? Would bimax be an option then?

nah, lower jaw expansion is way harder and wouldnt give u (or 99.9% of people) any functional benefits so its almost never done. everyone just augments the lower jaw in some sort of way unless they have something really wrong with them (u dont)
 
Yeah it that’s true but I read about Dr. Alfaro in Barcelona. And they do all sorts of jaw surgery, notably orthofacial surgery, which combines functional and aesthetics. Will link their website below. But that could be interesting to look at no?
Also do you happen to know someone who is an expert at creating individual hardmaxx plans?
