The penis is positioned in a flaccid state, and the glans, urethral opening, and underside are cleaned using an antiseptic applied with sterile gauze. The intended exit point is identified on the ventral surface of the glans, usually just behind the frenulum, and is marked with a sterile skin marker to ensure proper alignment with the urethral channel. A receiving tube is then positioned at this exit mark to stabilize the tissue and protect surrounding areas as the needle is advanced.

The hollow piercing needle, typically matched in gauge to the jewelry, is inserted directly into the urethral opening. It is advanced through the urethral channel until the tip emerges from the underside of the glans into the receiving tube. The motion should be steady and continuous to minimize tissue tearing. Once the needle has fully exited, the jewelry is introduced immediately, often by attaching it to the end of the needle and guiding it through the new channel in one smooth motion.

When the jewelry is in place, it is secured according to its design — for example, snapping the bead into position for a captive bead ring or tightening the threaded ends of a curved barbell. The area is then cleaned again with sterile saline to remove any blood or debris. Gloves are removed, single-use items are disposed of, and the piercing is considered complete.
