Incidentally, I've seen people in person who have had the Quadrangular LeFort and the results are still very underwhelming
So have you seen someone with that LeFort like you said earlier in the thread? Like that LeFort II without the nasion? Was it really underwhelming? Where did he have that done?

Yes, known as Quadrangular LeFort 2, basically Quad 1, but it extends to the infraorbitals.

Was it really underwhelming? Where did he have that done?
I saw two patients who underwent it. In both cases the maxilla was still hypoplastic. The side profile of one looked uncanny - like his midface was flat but it wasn't, probably because the nasal cavity was left behind. The surgery was performed in Vienna.

Lived there for a bit. What do you think of the result?
Other guys from that paper got better results, I think. This one side improved so much and looks so good, but the other views look basically the same, and it looks kind of bad under the eyes. You can see where the cut was made and where the end of the cut is on the sides. So the front honestly descended. But If he does implants and fatgrafting it will look great

There are no other results from that paper, frustratingly.

He'd have achieved similar results with LF1 + implants without the significantly increased invasiveness of cutting into the infraorbital foramen and orbits themselves.

I haven't seen any results of any of these eppley style facial implants posted anywhere. people always ask about them on here, but no one really ever sees how they turn out.

heres my face rn, front and side.

So my face is still kinda smushed in around the base of the nose. ive got the itachi lines and nasolabial folds. hopefully gunna take care of both of those. i want to augment the paranasals by about 4-6mm. i have 4.5mm inframalars rn, so id need to bump each of those up to around 10mm forward projection. also gunna try to make them a couple mm higher and outward. it will be paired with fat grafting and a mild face lift. so i think im gunna push for a shape like this, with red being 5mm and blue being 10.

MY MAIN QUESTION is, should the submalar region be skipped? i could have a much hollower cheek appearance by skipping it. would the skin look ok like that? such a big change to the surrounding area makes it pretty hard for me to gauge how the soft tissue will lay out around there.
Been following your journey since the beggining gawd damn man, you look awesome!, I think the change with the new implants would be great speacially considering the smile lines, when the design is ready could you upload them too?

Haven't decided yet, but leaning towards PEEK.

definitely looks a lot better in the after. nothing crazy though, coulda done lf1 with some add ons
Side profile yes, frontally, I wouldn't even notice a difference as a casual observer.

Probably just inframalars. I had a higher cult LF1 which probably renders piriform implants redundant but will think about it carefully.

I’m afraid he’s been duped into thinking that a brute force, functional mid-facial advancement surgery will make him good looking. Sadly, it doesn’t work that way.

I saw the result too. To be fair, there was still a lot of post op swelling but you’d wouldn’t even be able to tell that he’d undergone anything more than your typical bimax from the front without advance knowledge. Even the side profile change was marginal. Imagine, literally slicing your skull in half for a slightly more advanced nasion!

Inflated expectations meet cold hard reality.
A good projection rhino with paransal implants to augment would have vastly outperformed tylins result

Yeah, hard tissue surgeries usually don't have that breathtaking effect. But it's critical to get the base right and then augment the soft tissue.
Depends on the cost benefit analysis. I don't personally think undergoing a high-level osteotomy with coronal incisions, which cuts into the orbital and medial floor, and the slew of risks associated with that is worth it in 99%+ of cases.

This woman was a genuine candidate for the surgery by the way and the frontal improvement still wasn't great. Form follows function at the end of the day; your typical bimax candidate is unlikely to get an appreciably better result.

If you have normal nasal projection, better to stick to midfacial implants + LF1 tbh.

I haven't seen any new Tylin photos post op but I generally agree with you; implants + rhino could have probably achieved a cosmetically very similar result if not better without the insane invasiveness.

here u can see the current shape in motion so without frauding and all. lmk if u guys think the chin really needs addressing or not. it bothers me personally. obv shaved head nerf, growing it back out no worries

I've become very pilled on projection rhinos recently.

In my case, I have a very robustly developed midface however my nose lacks development, my radix is recessed, nasion and bridge are weak etc. As a result my midface appears to be less developed.

Also I've seen some insane results from chinese surgeons that mog anything a LF2 could do.

I've become very pilled on projection rhinos recently.

You unironically look very good from the front. I wouldn't even know you had maxillary hypoplasia; it's only the side profile where I can definitely see it.

I doubt the aesthetic result will be impressive, LF2 is not a cosmetic surgery.

Based on all the LF2 cases I've seen, I wouldn't bet on it. The implants are more likely to make a difference.

Imagine literally sawing your maxilla in half when you can just do this + implants. It's so crude.

Yeah definitely. He said he would do rhino and implants, but that wouldn’t be enough, so he went with L2 to bring his nose forward and then he will do midfacial implants

Cope max is the most overdone surgery on org. Blows my mind how many people think they need a bimax when all they need is a genio.

Agreed, maybe jaw angle implants. Literally an angular jaw puts you ahead of the vast majority of men and you don't have to fuck around with the orbits or nasal cavity to to get a good result.

I'm pretty pilled on jaw angles as a base for filler.

Get the width you need with the jaw angle and then get the angularity with filler.

The soft tissue predictability of jaw angles SUCKS

The masseter response is too unpredictable

i havent really, aside from people that really needed height. when the implants go downward below where the gonions were they seem to be able to design as crisp of an edge as they want. saw some on eppleys like this

They're limited but still have a far better return than going through, decomp, bimax and for <5 mm of LF 1 and mandibular advancement (the average .org user on here).
