INTRODUCTION
Genioplasty is a surgery that has the potential to ascend the side profile, and to a lesser extent frontal. The surgery involves cutting the chin bone (mandible) and moving it forward/backward and/or up/down to achieve the desired shape and projection. IF you are not massively recessed and have no medical issues associated with recession (eg. fucked bite or small airway determined via CT scan) and simply lack some chin projection then it can be an amazing surgery. However, if you are worried you might need more than just a genioplasty please consult with a maxillofacial surgeon and get multiple professional opinions.

SOMEWHAT COMMON NEGATIVES OF GENIOPLASTY
  • some permanent loss of nerves can occur in the chin, lip, or lower teeth area
  • bottom lip rolling in from the incision being cut too high or the stich being too tight
  • mentalis muscle dysfunction if it is not put back together properly
  • not extending mandible far enough by doing a conservative movement
All of these downsides can be solved by simply going to a good surgeon and doing your research. If you actually need DJS don't be a retard and just get genio.

PLANNING YOUR SURGERY
  • see if insurance can cover some or all of your surgery
  • find the right surgeon, some good questions to ask are: do they perform this regularly? are they able to do large movements? are they aesthetically knowledgeable?
  • do more research than just this guide, plenty of posts all over the internet about peoples experiences
  • prepare any scans you can prior to surgery, lateral cephalometric xray is a good starting point and easy to get
RECOVERING FROM GENIO

water tier stuff you will hear from most surgeons:
  • prepare to be unable to talk much for a few days
  • take minimum one week off work
  • sleep as much as you can, on your back for minimum first 3 nights
  • only clear liquids for first 24hrs
  • protein shakes etc. after 24hrs for at least 3-4 days normally with as many calories as you can and after that start introducing soft foods
  • walk as much as you can to reduce swelling
  • mouth wash after every meal to keep incision from getting infected
  • no heavy lifting for at least 3 weeks, most surgeons recommend 6 weeks
  • by end of week 2 most people (excluding family and those who see you regularly) wont notice you have had surgery
  • by weeks 3-4 you can start to have an idea of what the final result will be
lesser known methods of improving recovery:
  • bpc 157 0.5-1mg/day for the first 2-3 weeks post surgery increase recovery time and improve likelihood of regaining all your nerves, this will drastically cut down recovery time. I would use this for any facial surgery tbh.
  • topical vitamin k cream for bruising
  • high dose calcium, vitamin k2 and vitamin d3 for bone healing

Cost?
 
It really depends on the location you are in. Eg. some people in the examples got it for like 4k USD while others paid 10k+ I believe. Surgeries should not be something people cheap out on, which is why I didn't discuss it.

lesser known methods of improving recovery:
  • bpc 157 0.5-1mg/day for the first 2-3 weeks post surgery increase recovery time and improve likelihood of regaining all your nerves, this will drastically cut down recovery time. I would use this for any facial surgery tbh.
  • topical vitamin k cream for bruising
  • high dose calcium, vitamin k2 and vitamin d3 for bone healing

I'm planning on getting genio and rhino, and either buccal fat removal or cheekbone implant, the cheekbone implant would age better and my skin tighter ext. but buccal fat removal is cheaper and quicker to recover from and my natural cheekbones aren't that bad. 1. yes. even if you just extend horizontally with no vertical change your chin will appear taller because of the increased prominence. note that irl this will be far more apparent as you are rarely seen directly front on like in this photo.

this is the guy who got 15mm forward and 5mm down, even without the 5mm vertical lengthening it would still appear taller.

2. i probably shouldnt have included vertical augmentation the way i did, as implants are considered a form of genioplasty i wrote about it and then forgot to remove. you will still need screws for the proper vertical lengthening/shortening genioplasty.

3. 5mm is still half a cm which for the chin will be very noticeable and an improvement to correct minor lack of projection. im very good at morphs so if you provide a pic i can show you what it would look like here or in dms. also the reason women tend benefit from less is they have slightly smaller mandibles and do not need as much projection as men to look good.

also look at how insanely different the submental is afterwards, even semi conservative movements can provide much needed tightening of this area. OP would have never been able to achieve that no matter how much he leanmaxxed unless he got a neck lift to remove "excess" skin.

I don't have an underbite, but my maxilla's forward growth (unlike my mandible) is pretty poor, meaning that my mandibular teeth sit just behind my maxillary teeth, enough so that my bottom lip protrudes out further than my (already small) top lip making my maxilla look even slightly more recessed. if not genio, any other recommendations?

Bet, thanks for reply. It's not a huge issue anyways for now, when I'm leaned out it becomes less noticeable. Le Fort 1 would solve most of my problems but I'm just trying to see if there are alternatives that are less expensive/invasive.

The advancement is in line with the gonial angle, and not 90 degrees parallel to the ground right? That would add a bit of height.
Also, great thread + lifefuel motivation. Should have covered vertical genioplasty as well for completion - albeit getting that is more for frontal aesthetics than it is for profile shot in most cases.

    • Advancement: The chin can typically be moved forward by up to 10-12 mm.
    • Setback: The chin can be moved backward by up to 6-8 mm.
  • Vertical movement:
    • Upward: The chin can be moved upward by approximately 6-8 mm.
    • Downward: The chin can be moved downward by about 6-8 mm.
You can do both vertical and horizontal simultaneously by skilled surgeons.

it depends on the surgeon, theoretical max could be like 12mm forward 8mm down but not many could do that, strongly suggest finding a surgeon in your area who does large movements and asking them bro

SOMEWHAT COMMON NEGATIVES OF GENIOPLASTY
  • some permanent loss of nerves can occur in the chin, lip, or lower teeth area
  • bottom lip rolling in from the incision being cut too high or the stich being too tight
  • mentalis muscle dysfunction if it is not put back together properly
  • not extending mandible far enough by doing a conservative movement
All of these downsides can be solved by simply going to a good surgeon and doing your research. If you actually need DJS don't be a retard and just get genio.

PLANNING YOUR SURGERY
  • see if insurance can cover some or all of your surgery
  • find the right surgeon, some good questions to ask are: do they perform this regularly? are they able to do large movements? are they aesthetically knowledgeable?

  • prepare to be unable to talk much for a few days
  • take minimum one week off work

  • only clear liquids for first 24hrs
  • protein shakes etc. after 24hrs for at least 3-4 days normally with as many calories as you can and after that start introducing soft foods
  • walk as much as you can to reduce swelling
  • mouth wash after every meal to keep incision from getting infected
  • no heavy lifting for at least 3 weeks, most surgeons recommend 6 weeks
  • by end of week 2 most people (excluding family and those who see you regularly) wont notice you have had surgery
  • by weeks 3-4 you can start to have an idea of what the final result will be
lesser known methods of improving recovery:
  • bpc 157 0.5-1mg/day for the first 2-3 weeks post surgery increase recovery time and improve likelihood of regaining all your nerves, this will drastically cut down recovery time. I would use this for any facial surgery tbh.
  • topical vitamin k cream for bruising
  • high dose calcium, vitamin k2 and vitamin d3 for bone healing
