INTRODUCTION
Genioplasty is a surgery that has the potential to ascend the side profile, and to a lesser extent frontal. The surgery involves cutting the chin bone (mandible) and moving it forward/backward and/or up/down to achieve the desired shape and projection. Genioplasty can be performed alone or in combination with other facial surgeries at the same time, such as rhinoplasty or jaw surgery. The procedure should be done under general anesthesia (here is a favelatard talking about getting it under local and it is as horryfying as it sounds) and requires minimum one week of recovery time.

note: you may have realised that the best results are where their chin projection was extended to roughly align with the lips (if they are downgrown or have a recessed maxilla ignore), simply draw a line from the nose downards and as long as the side profile posture isnt fucked you can get a rough idea if the chin projection is where it should be
GENIO WITH JAW SURGERY
For the truly recessed, jaw surgery when accompanied by genioplasty can ascend sooo fucking much
10mm genio, 10mm left lower jaw/7mm right and had 6 or 7mm movement upper
lower jaw 5mm + 7mm genioplasty, this photo is only 10 days post op as well
30 mm lower and 4-6 (roughly) mm upper. Lefort 1 and BSSO for DJS, the genio was approximately 5-6mm

TYPES OF GENIOPLASTY
- sliding genioplasty (the king and most common): moves the chin forward without changing its height using a horizontal cut.
- wing genioplasty: widens the chin by making a horizontal cut and placing a graft.
- stair step genioplasty: advances and lengthens the chin using step-like cuts in the bone.
- box genioplasty: simultaneously advances and lengthens the chin using a rectangular cut.
- vertical reduction genioplasty: shortens the chin height by removing a wedge of bone.
- vertical augmentation genioplasty: lengthens the chin by inserting a graft or implant.
- t-shaped genioplasty: combines chin advancement, lengthening, and widening using a t-shaped cut.
- asymmetric genioplasty: corrects chin asymmetry by making different bone cuts on each side.
- reduction genioplasty: reduces chin size by removing a portion of the chin bone.

IF you are not massively recessed and have no medical issues associated with recession (eg. fucked bite or small airway determined via CT scan) and simply lack some chin projection then it can be an amazing surgery. However, if you are worried you might need more than just a genioplasty please consult with a maxillofacial surgeon and get multiple professional opinions.
SOMEWHAT COMMON NEGATIVES OF GENIOPLASTY
- some permanent loss of nerves can occur in the chin, lip, or lower teeth area
- bottom lip rolling in from the incision being cut too high or the stich being too tight
- mentalis muscle dysfunction if it is not put back together properly
- not extending mandible far enough by doing a conservative movement
PLANNING YOUR SURGERY
- see if insurance can cover some or all of your surgery
- find the right surgeon, some good questions to ask are: do they perform this regularly? are they able to do large movements? are they aesthetically knowledgeable?
- do more research than just this guide, plenty of posts all over the internet about peoples experiences
- prepare any scans you can prior to surgery, lateral cephalometric xray is a good starting point and easy to get
water tier stuff you will hear from most surgeons:
- prepare to be unable to talk much for a few days
- take minimum one week off work
- sleep as much as you can, on your back for minimum first 3 nights
- only clear liquids for first 24hrs
- protein shakes etc. after 24hrs for at least 3-4 days normally with as many calories as you can and after that start introducing soft foods
- walk as much as you can to reduce swelling
- mouth wash after every meal to keep incision from getting infected
- no heavy lifting for at least 3 weeks, most surgeons recommend 6 weeks
- by end of week 2 most people (excluding family and those who see you regularly) wont notice you have had surgery
- by weeks 3-4 you can start to have an idea of what the final result will be
- bpc 157 0.5-1mg/day for the first 2-3 weeks post surgery increase recovery time and improve likelihood of regaining all your nerves, this will drastically cut down recovery time. I would use this for any facial surgery tbh.
- topical vitamin k cream for bruising
- high dose calcium, vitamin k2 and vitamin d3 for bone healing
