I always wake up early in the morning or middle of the night
 
Pretty simple, don't use your phone 30 mins before sleep and use a blue light filter. Don't eat or drink 30 mins before sleeping and sleep in a cool room.

It's natural thing that you wake up in the middle of a night, before industrial revolution everyone slept like this. They used to go out for a walk or just have sex with partner and sleep for the next 4 hours. Just buy melatonin if you really can't sleep well. Don't drink water after 19:00, blue light filter, go for a walk before sleep, no phone before bed and walk a lot during the day, go swim with friends. Mark as solution
