Well, if you ask your typical or your most famous plastic surgeons out there, they'll say what you said because they even have silicone implants named after them.but doctors say it's a lot bigger than what required for silicone. also I couldn't find if peek/hard materials better for bone erosion/resorption in long term. how can you be so sure about that?
But it is true, however. In theory, you do need a "larger incision." But I don't see thousands of Maxillofacial complaining about that. Besides, Maxillofacial doctors are the ones that do this the most, not plastic surgeons.
Also, it is not only about the size of the incision but the technique and expertise of the doctor.
Erosion is caused by movement. If you fix the implant correctly and then you have a perfect fitting implant, there will be no erosion.
