I've made peace with removing my jaw angle implant. In June 2023, I got a customized medpor jaw angle implant (5mm in width with a slight dropdown). Ever since then, I feel it has been too bulky from the front. The side profile is objectively better, but I don't really care much about that. I've done quite possibly everything to slim it, including weight loss, buccal fat removal, buccinator myectomy, and perioral fat liposuction, but all were very limited. In March 2024, my surgeon shaved a few mm of my mid-mandible and part of the implant, along with a minor neck lift to reduce sagging, however, after 7 months, I still feel like it's too bulky. I prefer the less defined yet slimmer look of a triangular face than a square look. As someone who's almost 31, I'm tired of the process and would like it removed. Obviously, there's been a lot of money poured into this, but the waste of money really doesn't bother me, in all honesty. I tried and realized an implant just won't give me the angular look I thought I could get.

I also got a lip lift and rhinoplasty 4 months ago, so that's why I look different beyond the jaw.

You seem super knowledgeable in terms of surgeries. Where does your knowledge mainly come from? Forum?

What did u expect? also why no osteotomy?
I expected a more v-shaped jaw by slimming the area towards the chin and widening the posterior. While I suppose what I did helped, there is excessive fullness and a lack of significant definition. Also, the effectiveness of osteonomies are overblown and would at best give me very mild results, including a lot of downtime, since I didn’t have significant recession or deficiencies to begin with.

Many consultations with many doctors, plus I’ve also had many surgeries over the past 3 or 4 years. Some have been successful like my eyes, and some have not.. These forums are also helpful but sometimes they lack nuance and tend to parrot one or popular ideas on here without a firm understanding of what they’re actually talking about.

you don't think longer mandible would give better V?

Good job, I did the same with my jaw wraparound. Lip lift was a game changer for you by the way, looks great.

It has also made your mentolabial angle too deep from the side view.

Why would you get it removed? It looks good. You look like you have a high test natural chin, unlike alot of people who get surgery and I can tell they got it. If you want it to be a bit sharper just get really lean. But I wouldn't worry about it. It looks good.

Thanks, lip lift, while it definitely leaves a scar that requires laser and microneedling to fade, which is annoying, is superior long-term to fillers imo, especially for those whose lips curl inwards like mine. Fillers can’t really fix that. Also, I found that repeated lip filler injections rarely look as good over time due to built up scar tissue. At one point my upper lip was so heavy from filler, no one could see my teeth when I spoke.

I actually had a deep mentolabial angle to begin with. I haven’t touched my chin to cause that, but some fat grafting to the area can reduce the depth. Not a huge concern in the grand scheme of aesthetics, but more like a nice-to-have.
 
Why would you get it removed? It looks good. You look like you have a high test natural chin, unlike alot of people who get surgery and I can tell they got it. If you want it to be a bit sharper just get really lean. But I wouldn't worry about it. It looks good.
Thanks, I am currently experimenting with weight cutting. I’m fairly lean already at about 6’1 and 188lbs, but I’m pushing it to cut down to 175-180lbs. While I agree that it could make the jaw sharper, I think the tradeoff of eating up muscle mass and potentially looking like a skeleton limits how aggressive I pursue weight loss.

Have you had your implants removed or you are planning in removing it?

I also want to remove mine but im afraid of sagging and dehiscence

Correct just jaw angles. Despite design, it’s always difficult to account for how the implant will react with the soft tissues in your face. Moreover, even if the implant is placed correctly and has a good design, it can still look weird if it’s not proportionate to the rest of your facial features. In short, they’re hard to get right because they rely on so many things.

You need to consider how your jaw frontal angle will look after which will be effected by chin width, jaw width and ramus height
 
Which ones are before and which ones are after? Too many photos
 
I was gonna post this but then I started doubting myself, but then I was on the bus an saw a dude with super narrow jaw but massive masseters/soft tissue and it confirmed everything I was thinking.

I feel like the jaw is completely misunderstood.

I said this to my surgeon. I was like "Look at me from the front. This is the peak of my jaw, but that's not my gonion." He's like "No, your gonion is right here" and he reach under my jaw. So I was like "so you wouldn't try to push the peak of the jaw out by enhancing the gonion? You would want to push the masseter out." He said it's not even the masseter but all sort of different fibers and this and that, and started rattling off various anatomical terms, and explained where the implant sits and how it's supposed to push them all out. Compare that to the cave man notion of "push gonia out." He didn't even know what sort of implants I was talking about that try to expand the gonia, e.g. Eppley.

That's why I think so many jaw implant results are shit. They try to push the gonia out so that they becomes the widest point of the lower third (not the mandible, but the lower third including all soft tissue). That's why they all look good from the side, but not from the front. From the side, the gonion is what you perceive as the peak of the jaw, but as the face turns a kind of illusion takes place, and the peak of the soft tissue "takes over" as what you perceive as the peak of the jaw.

Look mane, your jaw is great. Let's say you get to 6 foot 1 178. You would feel skinny but In reality, if you are truly lean at that weight , you probably are more muscular looking than you think . Brad Pitt in fight club was like 155 lb or something. You may look smaller but just wear fitted t shirts.

Second, even if you just lost a solid 5-6lbs, the jaw would look sharper. Even if you don't cut, it still looks like a high test jaw. So don't bother getting it removed (unless it's for other reasons, such as you want to start boxing or something , or for your health for whatever reason). Have confidence in yourself and your jaw, cause it's definitely better than what people on here would lead you to believe.

I'm still on the fence, but thanks for the encouragement. That's extremely rare on a site like this. You make a valid point with the weight loss angle. I'll probably do that before ultimately deciding.
