Hey guys, I'm having a consultation soon with a jaw surgeon where he'll go over his plan for me including advancements and 3D simulation. I'm the typical SFS case with a short and compressed lower 3rd which causes soft tissue sagging all around my jaw and cheeks so I always have bloated cheeks, poor jawline definition, submental fat etc.

The surgeon is not known on this forum so I'm worried his proposed plan might be too conservative. What sort of movements would someone in my case need for the best aesthetic outcome? Also I'm considering getting buccal fat removal along with this and maybe submental lipo, good idea?

your surgeons priority should be the height of the vertical augmentation genioplasty, with the 5 pixels of ur upper jaw I see, a 2-3 milimeter advancement ( conservative i know ) mixed with a vertical advancement of 3-4 milimeters would be great, and a subtle horizontal slide can be added to your discretion. Of course, i'd have to use some advanced 3D simulation software to see how it'd all precisely look, which I dont have access to, but im sure your surgeon does, so your biggest priority is the short face syndrome, which again can be solved by a 3-4 vertical genio.

You are an ideal candidate for bimax. Downgrafting the mandible alone will really stretch out your soft tissue and provide some jawline definition.

Most of the celebrity surgeons promoted on .org have high visibility because of aggressive marketing or a good result with one particular user. Doesn’t mean they are necessarily better than local max facs.

Based on photos, I’d say you’ll need much more downgrafting of mandible than maxilla. Can’t really be specific on movements though.
