When can you not ascend?

I’m starting to doubt I can actually ascend.
By ascending I mean reaching 5PSL.
6PSL would be my dream.
Is there any features that must be good in order to ascend?
Is there some cases where its impossible to ascend even with surgeries.
I will dedicate my whole life trying to reach 5PSL
