How deformed do you have to be to get jaw surgery?

like I am not deformed looking but am in that grey area where my face didn't develop but i wouldn't be burnt alive in the medieval times, do surgeons ever refuse a patient?
i have both jaws significantly recessed and a recessed chin. is this surgery process elective? or is it like you can choose what to get and your surgeon will do what you wish....
I can only imagine the surgeon referring .e to a psychologist.
